Patient Rights & Responsibilities
Effective health care requires a special relationship between
health care provider and student - a partnership of trust, confidence,
and a mutual respect for rights and responsibilities.
You Have The Right To:
- Be treated with dignity and respect
- Know the names of the people caring for you.
- Receive explanations concerning your diagnosis, evaluation, treatment, and prognosis.
- Receive education and counseling.
- Have privacy and confidentiality of your medical records, and know that your records will not be released without your consent.
- Have a copy of your medical record.
- Request restrictions on release of records and disclosures.
- Review your medical records with a clinician.
- Participate in decisions regarding your health care, and consent to or refuse any care or treatment.
- Request or change a specific health care provider(s).
- Refuse to participate in experimental research.
You Have The Responsibility To:
- Be honest about your medical history.
- Be sure you understand your treatment plan.
- Follow health advice and medical instructions.
- Respect clinic policies.
- Report any changes in your health.
- Keep appointments, or cancel at least 24 hours in advance.
Family Planning Clients Have The Right To:
- Decide whether or not to have children, and when.
- Know the effectiveness, possible side effects, and health concerns of birth control methods.
- Participate in choosing a birth control method.

- When you have questions SPEAK UP
- When you have problems TELL SOMEONE
