Instructions for Completing Veteran Information Card (VIC)

You are required to complete a Veteran Information Card each semester you want to utilize your benefits.

Completing the VIC

  • Register for your classes via My SacState. You must be registered for your classes prior to completing your VIC.
  • Download the VIC.
  • The information requested on this form must be completed entirely (even if you think you have submitted it to the VA Office previously). Leaving sections blank will result in a delay in processing your certification. Please read all directions included on the form.
  • In the Class Schedule section, only indicate those classes in which you are officially registered. If you plan on adding classes or if your department is enrolling you in your classes, your registration must be official before you indicate this on your VIC. The classes you indicate on your VIC will be cross- referenced with the university's Student Information System. You may access your personal records through My SacState.
  • The information requested on this form will be used for VA business only. Therefore, information will not be shared with other Sacramento State departments, or outside entities. That also means that if you need to change your personal information with the university, you will need to complete that with the Office of Admissions and Records.
  • The form will "prompt" you to complete each section. Therefore, if a particular section is not relevant to your enrollment, type "n/a."
  • Entering your email address is compulsory-the VA Office needs your email address in order to communicate important information about your benefits. If you do not have an email address, type "n/a." If you would like to establish a SacLink account go to www.csus.edu/saclink for more information.
  • Remember to sign and date your form!

Submitting Your VIC and Other Paperwork

  • Please make sure to complete all paperwork that you need to complete to obtain certification. If you are a transfer student or changing or adding a major or minor, you will need to complete a "Request for Change of Program/Place of Training" Form.
  • If you do not have an Academic Plan on file, please complete this form and have it signed by your major advisor by your third term of certification. If you are unsure whether you have an Academic Plan on file, call the Sacramento State VA Office. It is your responsibility to make sure that you have all you paperwork submitted for certification. Incomplete files and/or paperwork will result in a delay of your certification processing!
  • If you are substituting classes that were not on your Academic Plan, you will need to complete and return a VA Student Course Substitution Form, signed by your advisor, with your VIC so that you can be certified for those classes.
  • If you are a Graduate Student, and are enrolled in undergraduate courses to satisfy pre-requisite requirements, the Sacramento State VA Office will send a Pre-Requisite Approval Letter to your Graduate Coordinator when you turn in your VIC. If your Graduate Coordinator does not approve these classes, you will not be certified for them.
  • You have several choices of how you may submit your VIC and other paperwork to the VA Office:

Bring it in Sacramento State Office of Veterans Affairs, Office of Admissions and Records, Lassen Hall (you will need to check in at the front window during drop in hours)

The VA requires that your original signature be on file for your certification to be processed. Therefore, emailed or faxed forms will not be accepted by the VA Office. Certifications will not be processed based on emailed or faxed forms.

Parking Permits

When your Vocational Rehabilitation Counselor sent your 22-1905 to the Sacramento State VA Office, your name was provided to the Student Financial Services Center so that you can receive a parking permit prior to the beginning of the semester. Your parking permit will be mailed to you. Please call Tamara Williams at 278-7434 if you do not receive your parking permit.

Books

In order to receive your books, you must submit a VIC to the Sacramento State VA Office. Depending on how you submit your VIC, your bookstore card will be given to you in person or will be made available for pick up at the Admissions and Records front counter. You will not be able to obtain your books without a bookstore card from the VA Office. The bookstore will not accept a 22-1905 in place of the bookstore card.

Fees

Once your VIC is submitted, then a copy of your completed 22-1905 is submitted to the Student Financial Services Center for billing purposes. Any other fees that you must pay (lab fees, class supplies fees, etc.) are your responsibility, and you will need to consult your Vocational Rehabilitation Counselor in regards to arranging a reimbursement.
