Instructions for Completing Undergraduate or Graduate Academic Plan

(Formerly "Major Clearance Form" and "Masters Plan Form")

The VA requires that you have an Academic Plan on file with the Sacramento State VA Office by or before your third term of certification. Your Academic Plan will be utilized for your enrollment certifications, and therefore, the courses you indicate on your Academic Plan must be as accurate as possible. If you take a class for your academic program that is not indicated on your academic plan, please submit a course substitution form, signed by your advisor with your VIC.

If you have already filed for graduation (undergraduate) or Advancement to Candidacy (graduate), please submit a copy of this document in lieu of the Academic Plan.

If you are unsure as to whether you have an Academic Plan on file, or, if you need assistance completing this form, please call the VA Office at (916) 278-6733. It is your responsibility to have a complete file for certification. If your file is incomplete, your enrollment certification will not be processed.

Instructions:

  1. Download the Undergraduate or Graduate Academic Plan
  2. Complete all sections. Do not list GE courses unless GE courses can be used to satisfy your major/concentration/minor requirements.
  3. List all courses completed (with grade), in progress and to be completed.
  4. If you have more than one major, minor, or concentration, submit a separate Academic Plan for each.
  5. Your major advisor must approve your major and concentration courses and your minor advisor must approve your minor courses.
  6. Please sign this form.
  7. Make a copy of this completed form before returning it to the VA Office.
  8. Please return this form with your VIC and other supporting documents in your 3rd term of certification.
